Applying to the Institutional Animal Care and Use Committee (IACUC)
The IACUC committee meets once per month on an as-needed basis. Reviews are typically completed through the Designated Member Review process (see AWP: How to Apply to the IACUC for more information).
Research Report Form for renewal request or closure
If you lost your Research Report Form or need to close a study earlier than the expiration date, you can submit a Protocol Closure through the InfoEd portal. This will be linked to your approved application.
Change (Amendment) requests
The IACUC must be notified in writing of any modifications to the approved research protocol. This is completed by submitting a Modification form through InfoEd. Such changes must be approved by the IACUC before they are implemented. Changes unrelated to study procedures must also be reported in order to keep the IACUC's file current. If clearly delineated as administrative in nature, the report of such changes will be processed by IACUC staff administratively.
A pre-formatted form is not currently available online. Submit a memo, signed by the PI, directly to Research Protection Programs.
